Medical Form Information Please bring this information to registration DO NOT mail, e-mail or fax these forms/letters in ahead of time
Medical Forms: The Naval Academy Athletic Association is now asking our campers to provide a complete and valid physical examination that was performed within one year of the start of camp.
This would be a form from your son’s doctor, much like ones you would need to submit to a school. If your doctor does not have a specific form, you can substitute this with a letter from the physician’s office stating the camper is in good physical health and able to participate in a rigorous athletic camp. Please include any allergies and/or daily medications.
Please bring this information with you to registration on the first day of camp. DO NOT mail or fax these forms/letters ahead of time. Another e-mail reminding everyone to obtain these medical forms will be sent out within a month of the start of camp.
